终止原因：成都嘉星宁医疗科技有限公司因“投标产品未提供检测报告”，未通过有效性、完整性和响应程度审查。四川荣悦昕健康管理有限公司因“冲击波治疗仪无检测报告”，未通过有效性、完整性和响应程度审查。 成都苏锦医疗器械有限公司因“冲击波治疗仪检测报告未提供要求的参数”，未通过有效性、完整性和响应程度审查。 根据“竞争性谈判文件 第六章 评审方法 2.12终止谈判采购活动。出现下列情形之一的，采购人或者采购代理机构应当终止竞争性谈判采购活动，发布项目终止公告并说明原因，重新开展采购活动：（3）在采购过程中符合要求的供应商或者报价未超过采购预算的供应商不足3家的。”之规定，本项目符合要求的有效供应商不足3家，故本项目废标。
